Home Care Packages

Diversicare’s Home Care Packages (HCPs) allow you to have complete control over your care choices, so you can get the support you want to enjoy life and maintain good health and wellbeing.

Under the Consumer Directed Care (CDC) model and as a Diversicare client, you are encouraged to identify your personal goals which can include increasing independence, wellness and re-ablement.

We will partner with you to help you make choices on who will provide your services and how and when they will be delivered.

These services are provided by Diversicare’s trained bilingual home care workers and take into account culture, language, and the needs of you and your family.  Our carers speak a variety of languages and these include Spanish, German, Yugoslav, Samoan, Dutch, Vietnamese, Polish, Croatian and Chinese.

To receive Home Care Packages, assessment and approval by the Aged Care Assessment Team (ACAT) is required.

The five steps outlined are:

  • Step 1: Check your eligibility
  • Step 2: Find a Home Care Package provider
  • Step 3: Work out the costs
  • Step 4: Accept a Home Care Package
  • Step 5: Begin your services

If you have higher care needs and are considering accessing home care, you will first need to contact My Aged Care by calling 1800 200 422 or going to the My Aged Care website link below.

If your care needs indicate you might need a Home Care Package, My Aged Care will refer you to an Aged Care Assessment Team (ACAT) to complete an assessment. Five Steps to My Aged Care


Through the introduction of an individualised budget, CDC provides greater transparency to you about what funding is available under the package, what you need to contribute and how those funds are spent.

Diversicare provides three levels of care packages:

  • Home Care Level 1 – supporting people with basic care needs
  • Home Care Level 2 – supporting people with low care needs
  • Home Care Level 3 – supporting people with intermediate care needs
  • Home Care Level 4 – supporting people with high care needs 

Brokerage services for various language groups are also available. Private Fee for Service provision may be more suitable for you if you are a part or fully self-funded retiree.
